Transformation Lead appointed to Dufferin-Caledon’s Ontario Health Team

ORANGEVILLE, ON – February 16, 2021 – The Hills of Headwaters Collaborative Ontario Health Team is pleased to announce the appointment of Alexander (Alex) Lim as its Transformation Lead.
In December 2019, the Hills of Headwaters Collaborative was one of the first of 24 Ontario Health Teams in the province to implement a new model of organizing and delivering health care. The province now has 42 teams that will cover 86 per cent of the population in Ontario, when they are at maturity.
Lim is a seasoned and visionary health system leader certified in project and change management, with over 10 years of health sector experience at the regional, provincial and national levels. Lim holds a Master of Public Health from the University of Waterloo and Master of Law (Health Law) from Osgoode Hall Law School.
“I am very delighted to be joining the Hills of Headwaters Collaborative Ontario Health Team and look forward to leading the OHT as we embark on this next phase of planning and implementation,” remarked Alex Lim. “My first priority will be meeting as many members of the collaborative as I can to get a full picture of the community and how I can serve it best.”
As Transformation Lead, Lim will lead the creation and implementation of an organizational framework which supports improved models of care that are patient-centered, efficient and simplified for patients and providers within the region.
“This is a tremendous first step in establishing a dedicated team to support the efforts our working groups, committees and councils. Alex will help us continue to build on the important work already underway,” said Kim Delahunt, President and CEO, Headwaters Health Care Centre and Co-Chair, Hills of Headwaters Collaborative.
“While it certainly is an unusual time to be joining a new team, we look forward to supporting Alex in his transition. We have full confidence in his ability to help us navigate our collaborative response to the pandemic and connect our regional health and care systems,” added Lianne Barbour, Executive Director, Dufferin Area Family Health Team and Co-Chair, Hills of Headwaters Collaborative.
The Hills of Headwaters Collaborative, much like other Ontario Health Teams, has had to pivot in response to the COVID-19 pandemic. The Collaborative has created a unified response and remained steadfast in its commitment to improve the entire health, community, home and social and human services experience for the people in Dufferin-Caledon.
