Hills of Headwaters Collaborative Ontario Health Team Welcomes Tracy Coffin, New Executive Director

Hills of Headwaters Collaborative (HOHC) Ontario Health Team (OHT) is pleased to announce that Tracy Coffin will be joining HOHC as its Executive Director effective today, September 6, 2022.

Reporting to HOHC’s Anchor Council’s co-chairs, the Executive Director will provide leadership to advance HOHC’s system transformation strategy. Tracy will work closely with HOHC’s partners to improve the health care journey of Dufferin-Caledon’s community members.
