Together with our partners, Hills of Headwaters Collaborative is building a foundation to better support residents in the community.
While we are in the early days of building our team, with the help of residents, patients and partners, we have identified and focused on these initial priority areas:
- COVID-19 Response
- Bringing together our mental health and addictions services
- Creating a collaborative palliative care team
- Providing supports for unpaid caregivers
As we begin to emerge from the pandemic, we will continue our foundational work and plan next steps to strengthening health care connections around the needs of Dufferin-Caledon residents to help us:
- Ensure patients, families and caregivers voices are heard and that we understand what is most important to them
- Share information across digital health systems
- Create and nurture long-standing relationships
- Build trust between our partners
- Place an emphasis on teamwork
