Frequently Asked Questions about Caregivers in the Hills

Caregivers in the Hills is a program developed by the local Hills of Headwaters Collaborative Community Wellness Council to assist caregivers in Dufferin and Caledon who provide essential day-to-day support to a loved one. The focus of the program is to recognize and validate the role of the unpaid caregiver as a pivotal partner in the health and social care team.

It’s easy! Fill out the application form, provide us with a photo and receive your badge (and a nice welcome package too!).

We are working with our Hills of Headwaters collaborative partners to create awareness of the program and to encourage partners to inform caregivers of the program.

If you are an unpaid Caregiver, you qualify to be a member of the Caregivers in the Hills program. We will provide support to complete the application if needed, online or by phone. Caregivers are provided with an identification badge to distinguish their role as caregivers within the local health and social care community.

When we asked caregivers what they need, they told us they wanted “recognition and validation of my role”. Therefore, the Caregivers in the Hills badge will act as a simple visual way of identifying unpaid caregivers. The badge will allow health and social care teams to recognize caregivers as partners in the care of their loved one. We are working with health professionals to make them aware of the badge and to recognize and validate the caregiver’s role.

The Caregivers in the Hills program is funded by Hills of Headwaters Collaborative and is FREE to members. The program has been developed to enable unpaid caregivers to be identified, validated and supported while navigating the health care system.

Each applicant will be contacted by our team via their preferred method of contact. We would like to learn more about your experience as an upaid caregiver to better assist you and link you to any applicable resources.

To apply to be a member of Caregivers in the Hills, you may reside anywhere; however, your loved one or the individual(s) you care for must reside in Dufferin County or Caledon.
