What is a Primary Care Network (pcn)?

Primary care and family physicians are the foundation of our health care system. In order to create a connected health care system in Dufferin & Caledon, the Hills of Headwaters Collaborative Ontario Health Team (HOHC OHT) is committed to collaboration and engagement with primary care providers through the creation of a Primary Care Network (PCN). The PCN will connect, integrate and support primary care providers within the HOHC OHT to improve the delivery and coordination of care for patients and improve primary care provider experience.

The HOHC OHT Primary Care Network Leadership Council has been established to represent the voice of the primary care provider community. The PCN will provide collaborative advice and leadership for the planning and implementation of coordinated health care in the region. 

The PCN will be defined as all those providing primary care in the Hills of the Headwaters geography. It is expected that the appointed representatives will represent the best interest of all primary care providers within the HOHC OHT and champion primary care informed initiatives with mutually agreed upon accountabilities.

what we are working on for you

The PCN is actively working with the HOHC OHT and its partners to create a coordinated access pathway for specialist referrals in the Dufferin & Caledon geography. Coordinated access will ultimately support primary  care providers in decreasing administrative work related to specialist referrals, making it easier to provide improved access to care.

The PCN will be hosting regular Continuing Medical Education (CME) opportunities for members, in order for them to maintain, develop, and increase their knowledge, skills, and performance.

MEET YOUR LEADERSHIP TEAM

Dr. Michael Gagnon, PCN Co-Lead

Doctor Michael Gagnon has been practicing family medicine in Bolton since 2007 and has been the Medical Director of Bethell Hospice since 2012. He graduated from the Royal College of Surgeons in Ireland in 2004 with Honours and went on to complete his residency in Family Medicine in Abington Pennsylvania where he took an interest in Palliative care. Dr Gagnon teaches family medicine residents from both the University of Toronto and Toronto Metropolitan University.

Dr. Sonya Caruso, PCN Co-Lead

Sonya Caruso is a family physician at North Dufferin Wellness Centre, in Shelburne, practicing comprehensive family medicine. She completed her medical degree at the University of Limerick in Ireland and residency at the University of Saskatchewan, in Saskatoon. She has a special interest in women’s health and office procedures. She also holds hospital privileges at Headwaters Healthcare Centre, where she provides ambulatory care and surgical assist.
